Position Goal

The HIM Tech III is responsible for receiving and reconciling all records from patient care areas. The HIM Tech III is also responsible for reviewing all documents for completeness and indexing the documents into the EHR.

Position Responsibilities:

• Performs initial, interim and final analysis on assigned charts using established departmental policies and procedures.
• Retrieves and reconciles inpatient and outpatient records from patient care areas.
• Reviews and prepares files and documents based on the requirements for conversion to include but not limited to arranging records in date order, removing duplicate copies, removing staples and clipping multi-page documents.
• Reviews the scanned document images electronically to check for accurate and complete scanning of all pages within the document. A 100% verification of each scanned page image shall be included in the review to determine the image quality. Document re-scanning will be required if the images scanned are considered poor quality.
• Performs final analysis of completed charts according to established departmental guidelines.
• Ensures transcribed documents are available on a daily basis.
• Enters deficiencies into the computer and prints out a deficiency record for physician use.
• Participates in notification of physician of deficiencies per departmental policies, and aids physicians in the completion of records.
• Performs duties as necessary in the absence of staff to prevent backlogs in workflow.
• Provides back-up coverage for Release of Information.
• Performs other duties as assigned by Director of HIM.

Experience: 

Preferred:

• Three years of Health Information Management experience preferred.

Education:

Required:

• High School Diploma or equivalent

Preferred:

• Completion of some coursework in HIM or an advanced degree in a related field

Special Qualifications:

Required:

• Strong attention to detail
• Excellent communication skills (written and verbal)

Preferred:

• Previous scanning experience
